Updated Information:

Dr. Robert K. Spees stopped working at our office location September 30, 2025 with the closing of Flagship Health Colorado Springs.

That office transitioned to Colorado Physician Partners - Palmer Park. 

Dr. Robert K. Spees recommends that his patients stay up to date with their health concerns with Dr. Carolyn Shay Moore at CP2 -Palmer Park.

The telephone number for CP2- Palmer Park is 719-597-4200. 

Please call that phone number to request medical records if needed.

Update for parents of Pediatric Patients seen from 2002 to 2023:

Dr Annelise Spees, Developmental-Behavioral Pediatrics, retired from patient care October 20, 2023.

To request medical records for past visits for your child, please:

• complete the HIPAA Medical Records Release Permission form. The form must be completed by the patient if over 18 years old or the parent/guardian.

• Make a check payable to "DMGCS" for $5.00 to cover record retrieval costs.

• Print, complete and sign the Release consent as well as the Medical Record Request form. 

• Mail the check and form to DMGCS, 1022 Fortino Blvd #8597, Pueblo CO 81008.

• Provide a valid email address to receive the record documents as attachments.

Please note:

1. If a patient is older than 28 years or has not been seen by Dr. Annelise Spees within the past 10 years, the records are no longer available. 

2. As of October 20, 2023, we are unable to create new documentation of current diagnoses or function of a patient. Please contact the patient's current medical or therapy providers for ongoing assessments and diagnoses.

3. Records of past evaluations, testing, and diagnoses will be emailed as attachments. We do not have fax capability since the office closure.
